Providence at Home is a growing division of Providence Life Services , which provides a continuum of healthcare, focused on senior living. Providence at Home is seeking a Home Health Registered Nurse (RN), Full Time in the Palos Heights, IL area within a 25 mile radius. Providence at Home is a Joint Commission accredited and celebrating 30 years of home-based services.
Schedule Available: Monday - Friday during normal business hours. On-call duties receive additional compensation. Home Health Visits happen in a 25 mile radius within Palos Heights, IL. Mileage reimbursement is $0.76/mile.

Summary of essential job functions:
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(s).
- Regularly re-evaluates patient nursing needs.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures.
Requirements
- Must be a Licensed Registered Nurse (RN) for the State of IL.
- Minimum of 1-2 years’ experience, at least one of which in the area of home care nursing, hospice, ER or Med surg.
- Must be CPR certified.
- Driver’s license and good driving record
- Reliable transportation to travel to patients in the community
