Shift Availabilities :
- Full-Time - $5,000.00 Sign-on Bonus*!
- Saturday and Sunday (weeknight/on-call)
- PRN Evening/Weekend (on-call)
As a RN Case Manager, you will :
- Visit with patients in their homes or nursing facility setting, providing assessment, teaching and skilled care
- Work with and receive support of other professionals to problem solve and update the patient's plan of care
- Other duties as assigned

Requirements
Qualifications :
- Valid Registered Nurse (RN) license in the state of Nebraska
- Excellent computer skills
- Excellent organizational and communication skills a must
- Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance
