What You’ll Do

- Follow each patient’s individualized care plan and physician orders.

- Monitor patient progress and report changes or safety concerns.

- Teach patients and caregivers self-care techniques.

- Partner with the RN Case Manager to update care plans as patient needs change.

- Document medication regimens and maintain medication profiles according to agency guidelines.

- Communicate with physicians, patients, families, and the care team to coordinate care.

- Help prevent avoidable hospitalizations and emergency room visits through proactive nursing care.

What You’ll Need

- Current state license as a Licensed Vocational / Practical Nurse.

- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.

- At least 1 year of experience in a clinical care setting.

- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

- Hospice or home health experience preferred.
