RN Triage Nurse - Primary Care

We are seeking a compassionate and experienced Triage Nurse (RN) to join our growing healthcare team. This position plays a vital role in ensuring patients receive timely, appropriate care by assessing symptoms, prioritizing needs, coordinating follow-up, and serving as a key clinical resource for patients and providers.

What You'll Do

- Conduct telephone and electronic triage for patient concerns and symptoms.

- Assess urgency of patient needs using nursing judgment and clinical protocols.

- Provide patient education and self-care instructions.

- Coordinate care with providers and clinical staff.

- Process medication refill requests according to clinic procedures.

- Review and communicate laboratory and diagnostic results as directed.

- Schedule patients appropriately based on clinical need.

- Document all patient interactions accurately in the electronic health record.

Requirements

- Active RN license.

- Current BLS certification.

- Minimum one year clinical nursing experience.
Preferred:

- Ambulatory care, family practice, internal medicine, urgent care, or triage experience.

- Experience with electronic health records.

- FQHC or community health center experience.
